  1. Birth Certificate (Original or Certified Copy)
  2. Proof of Physical Examination within the last year on yellow form
  3. Immunizations record (Blue form)
  4. Proof of Residence (Deed Property,Tax Bill or Current lease)
...obtain free or reduced lunch for your child: Ask anyone in the office for a free/reduced lunch application.
Fill it out and wait a short time to get an answer.
...become a classroom volunteer or chaperone a field trip :
